The term itself was first used in the Daily Mirror and referred to a concert at the London Palladium on October 13, 1965, during which the noise in the hall was deafening throughout. However, this also applied to earlier and later Beatles concerts, where teenage girls screamed hysterically, so loud that the music was practically inaudible.
As the Beatles themselves later admitted, even they couldn't hear themselves, which is why their live performances of their songs were faster than their album recordings. Ringo Starr admitted that sometimes he would enter at the wrong time because he couldn't hear the other musicians, and sometimes they would even mimic their singing, as it was impossible to discern anything in the noise.
It all ended on August 29, 1966, when The Beatles gave their final concert in San Francisco. Incidentally, it lasted less than an hour.
